The patient is sensitive to noise, easily startled, and prone to irrational fears, often manifesting as a feeling of having committed a crime.
Zincum Metallicum acts primarily upon the cerebro-spinal nervous system. It is indicated in states of profound exhaustion, where the vital powers are too weak to develop eruptions or bring acute diseases to a crisis. It is characterized by a specific 'fidgetiness' of the feet, a state of hyperesthesia, and a general deficiency in nerve power leading to paralysis or choreic movements. Aggravation: From wine, even in small quantities, after eating, from touch, noise, and during the night. Amelioration: From the appearance of discharges, such as the menses, or the development of skin eruptions; also from pressure and open air.
Vertigo is experienced with a sensation of falling to the left side, often accompanied by intense pressure at the root of the nose and a feeling of heaviness in the occiput. Vertigo is worse when walking in the open air.
Headaches are deep-seated, often occipital, and associated with brain fatigue. There is a sensation as if the head would split, especially after drinking wine or during the menses. The scalp is sensitive to touch, and the pain is frequently relieved by the return of normal discharges.

In the urinary system, there is involuntary micturition during coughing or walking. Menses are often too early, too profuse, and accompanied by severe ovarian pain, which is relieved when the flow begins.
The most prominent symptom is the constant, restless motion of the feet. There is a sensation of trembling, weakness, and numbness in the limbs. Joints may be swollen or rheumatic, with sharp, stabbing pains that move from place to place.
The back is the seat of severe burning pains, especially along the spine. There is extreme weakness in the lumbar region, with a sensation as if the back would break. Stiffness of the neck and shoulders is common.
The skin is dry and inactive. Eruptions, such as eczema or boils, are slow to appear or have been suppressed, leading to internal nervous symptoms. There is intense itching, especially in the evening, and a tendency to varicose veins.
Sleep is disturbed by vivid, anxious, or frightening dreams. The patient may startle during sleep and awaken feeling unrefreshed. There is a tendency to talk or cry out during sleep due to the underlying nervous irritability.
The patient is generally chilly, yet cannot tolerate the heat of a room. There is a significant lack of vital reaction; the body struggles to complete the healing process in acute diseases. All symptoms are markedly worse after the consumption of alcohol, particularly wine.
Sulphur and Calcarea Carbonica are often complementary. Sulphur helps bring out suppressed eruptions, while Calcarea Carbonica addresses the underlying constitutional weakness and metabolic dysregulation, helping to complete the curative process initiated by Zincum.
Nux Vomica is often considered inimical if given concurrently or too closely; it may aggravate the nervous irritability. Chamomilla and Ignatia should be used with caution, as they may disturb the action of Zincum in sensitive patients.
