Ankylosing spondylitis (AS) is a chronic, systemic inflammatory rheumatic disease primarily affecting the axial skeleton, particularly the sacroiliac joints and the spine. It is characterized by inflammatory back pain, progressive stiffness, and in advanced stages, the potential for bony fusion of the vertebrae (ankylosis). As a member of the seronegative spondyloarthritides family, it typically lacks rheumatoid factor but has a strong genetic association with the HLA-B27 antigen.
The exact cause of ankylosing spondylitis remains unknown, but it is considered a polygenic inflammatory disease. The strongest known factor is genetic; specifically, the HLA-B27 gene is present in approximately 90% of affected Caucasian patients. However, only a small percentage of people with the gene develop the disease, suggesting environmental triggers. Theories include "molecular mimicry" involving gut bacteria (like Klebsiella) and alterations in the gut microbiome that trigger an immune response in genetically susceptible individuals.
The primary pathological site is the enthesis—the point where tendons, ligaments, or joint capsules attach to bone. The process begins with immune-mediated inflammation (enthesitis), characterized by the infiltration of T-cells and macrophages and the overproduction of pro-inflammatory cytokines, specifically Tumor Necrosis Factor-alpha (TNF-α) and the IL-23/IL-17 axis. Chronic inflammation leads to erosion of the bone. During the healing phase, new bone (syndesmophyte) forms. Over time, these syndesmophytes bridge the gaps between vertebrae, leading to spinal fusion and the characteristic "bamboo spine."

The course of AS is highly variable. Many patients maintain functional independence and a full work life. However, up to 30% may develop significant disability due to spinal fusion or hip involvement. Early initiation of biologic therapy has significantly improved long-term outcomes and quality of life.
Primary prevention is not currently possible. Secondary prevention focuses on early diagnosis using MRI and HLA-B27 testing to initiate therapy before irreversible bony fusion occurs.
