Budd-Chiari Syndrome (BCS) is a rare, life-threatening disorder characterized by the obstruction of hepatic venous outflow, ranging from the small hepatic venules to the junction of the inferior vena cava (IVC) with the right atrium. This obstruction leads to increased sinusoidal pressure, portal hypertension, and progressive liver injury.
BCS is primarily caused by an underlying hypercoagulable state.
The obstruction of hepatic venous drainage leads to severe sinusoidal congestion and post-sinusoidal portal hypertension. At the cellular level, this congestion causes localized hypoxia, pressure atrophy, and centrilobular (Zone 3) necrosis of hepatocytes. Over time, chronic congestion stimulates fibrogenesis, leading to cirrhosis. Compensatory hypertrophy of the caudate lobe often occurs because its venous drainage bypasses the main hepatic veins, draining directly into the IVC.

Contrast-Enhanced CT or MRI of the Abdomen: Purpose: Characterize vascular anatomy and parenchymal perfusion. Typical Findings: Non-visualization or narrowing of hepatic veins, patchy "nutmeg" liver enhancement, caudate lobe hypertrophy. Clinical Importance:* Confirms diagnosis, defines the extent of IVC involvement, and rules out compressive masses.
Prognosis has improved significantly with stepwise management (anticoagulation $\rightarrow$ angioplasty $\rightarrow$ TIPS $\rightarrow$ transplant).
