Buerger's disease, or thromboangiitis obliterans (TAO), is a rare, non-atherosclerotic, segmental inflammatory vasculitis primarily affecting the small and medium-sized arteries and veins of the extremities. It leads to progressive occlusion of blood vessels, causing severe ischemia, pain, ulceration, and ultimately gangrene of the fingers, toes, hands, and feet. Buerger's disease is rare, with a prevalence estimated at 12.6 to 100 cases per 100,000 population in regions with high tobacco use. It predominantly affects males (historically >90%), typically between 20 and 45 years of age. While historically more prevalent in men, the incidence in women has increased with changing smoking patterns. Purpose: Assess blood flow and identify arterial occlusions in peripheral vessels.
Typical Findings: Reduced or absent flow in distal arteries, increased resistance.

The prognosis is highly dependent on complete and permanent cessation of tobacco use. With sustained abstinence, disease progression often halts, symptoms may improve, and limb salvage rates are significantly higher. Continued tobacco use leads to inevitable progression, recurrent ischemia, and high rates of amputation (up to 40-50% in 5-10 years). Mortality is not directly due to Buerger's, but complications like severe infection.
