Celiac disease is a chronic, systemic, autoimmune-mediated enteropathy triggered by the ingestion of dietary gluten (a protein found in wheat, rye, and barley) in genetically susceptible individuals. It primarily affects the small intestine, leading to mucosal inflammation, villous atrophy, and subsequent nutrient malabsorption.
Upon ingestion, gluten is partially digested into proline- and glutamine-rich peptides like the 33-mer gliadin peptide. These peptides cross the lamina propria of the small intestine, where they are deamidated by the enzyme tissue transglutaminase (tTG). Deamidated gliadin possesses a high affinity for HLA-DQ2 and HLA-DQ8 molecules on antigen-presenting cells. Presentation to CD4+ T helper cells triggers an inflammatory cascade characterized by the release of pro-inflammatory cytokines (such as interferon-gamma and IL-15). This process drives intraepithelial CD8+ T-cell infiltration, crypt hyperplasia, and progressive enterocyte destruction (villous atrophy), diminishing the absorptive surface area of the duodenum and jejunum.

The majority of patients achieve complete symptomatic relief and mucosal healing within 3 to 12 months of initiating a strict gluten-free diet. The long-term prognosis is excellent with dietary compliance.
There are no established primary prevention strategies. Secondary prevention focuses on screening high-risk individuals (e.g., type 1 diabetics) to prevent long-term complications.
