Churg-Strauss Syndrome, clinically known as Eosinophilic Granulomatosis with Polyangiitis (EGPA), is a rare systemic necrotizing vasculitis characterized by eosinophil-rich inflammation involving multiple organ systems, most notably the lungs, skin, and peripheral nerves. It typically evolves through three phases: prodromal (allergic), eosinophilic, and vasculitic.
The exact cause remains idiopathic. It is thought to involve a complex interplay between environmental triggers (e.g., allergens, medications like leukotriene receptor antagonists) and an underlying genetic predisposition. It is not strictly hereditary but may involve specific HLA-DRB4 gene polymorphisms.
EGPA is characterized by Th2-mediated immune responses leading to tissue infiltration by eosinophils. Cytokines such as IL-5 stimulate eosinophil survival and activation, resulting in degranulation and release of cytotoxic proteins (e.g., major basic protein), which damage endothelial cells, leading to systemic vasculitis and granuloma formation.
