Cirrhosis is the late, irreversible stage of progressive hepatic fibrosis characterized by distortion of hepatic architecture, formation of regenerative nodules, and vascular reorganization. It leads to progressive hepatic dysfunction and portal hypertension.
Chronic parenchymal injury triggers inflammatory cascades and activates hepatic stellate cells (HSCs) in the space of Disse. Activated HSCs transdifferentiate into myofibroblasts, secreting excess extracellular matrix components (primarily Collagen Types I and III). This progressive fibrogenesis disrupts hepatic sinusoidal fenestrations (sinusoidal capillarization) and compresses hepatic venules, increasing vascular resistance (portal hypertension). Loss of functional hepatocyte mass impairs synthetic, metabolic, and clearance capacities.
Abdominal Ultrasound: Purpose: Initial diagnostic screening. Typical Findings: Nodular liver contour, splenomegaly, ascites, and patent or reversed portal vein flow (reverse flow is hepatofugal). Clinical Importance: Screens for early-stage Hepatocellular Carcinoma (HCC) and portal vein thrombosis.
Transient Elastography (FibroScan): Purpose: Non-invasive quantification of liver stiffness. Typical Findings: Elevated liver stiffness measurement (typically >12.5 kPa indicating cirrhosis). Clinical Importance: Correlates directly with the degree of hepatic fibrosis.
Prognosis depends on the presence of decompensating events (ascites, variceal bleeding, encephalopathy).
