Chronic cortisol excess binds to systemic glucocorticoid receptors, accelerating gluconeogenesis, protein catabolism, and lipolysis. Central adiposity occurs via stimulation of lipoprotein lipase. Skin thinning and purple striae result from dermal collagen breakdown. Mineralocorticoid receptor cross-activation by excess cortisol induces sodium retention, volume expansion, and arterial hypertension.
Pituitary MRI (with gadolinium contrast): Purpose: Evaluate for ACTH-secreting pituitary micro- or macroadenomas. Typical Findings: Hypoenhancing lesion in the anterior pituitary. Clinical Importance: Identifies the source for transsphenoidal surgical planning.
Abdominal CT Scan: Purpose: Evaluate bilateral adrenal anatomy. Typical Findings: Unilateral lipid-rich nodule (adenoma) or bilateral diffuse hyperplasia. Clinical Importance: Essential for planning adrenalectomy in ACTH-independent disease.
Exogenous Cushing's is prevented by utilizing the lowest effective dose of corticosteroids for the shortest duration possible, tapering the medication under medical supervision.
