Dry Eye Syndrome (DES) is a multifactorial disease of the ocular surface characterized by a loss of homeostasis of the tear film. It is accompanied by ocular symptoms such as irritation, redness, and visual disturbances resulting from tear film instability, hyperosmolarity, and inflammation of the ocular surface.
DES is primarily caused by tear deficiency (aqueous-deficient) or excessive evaporation (evaporative). Factors include Meibomian gland dysfunction (MGD), aging, hormonal changes (post-menopause), autoimmune conditions (Sjögren’s syndrome), environmental exposure (low humidity, wind), and prolonged digital device usage.
The core mechanism is tear hyperosmolarity, causing epithelial cell damage and apoptosis. This triggers a pro-inflammatory cascade, releasing cytokines into the tear film, which further destabilizes the lipid layer, leading to a vicious cycle of desiccation and chronic inflammation.
Global prevalence ranges from 5% to 50%. It is more common in women, older populations (>50 years), and those living in arid climates.
