Empyema is primarily caused by bacterial infection of the pleural space. The most common primary event is bacterial pneumonia, where infection extends from the lung parenchyma into the pleural space. Other causes include:

  • Trauma: Penetrating chest trauma.

Differential Diagnosis

  • Complicated Parapneumonic Effusion: An infected pleural effusion that is not yet frankly purulent. While also requiring drainage, its pleural fluid pH might be slightly higher (>7.2) and Gram stain/culture may be negative initially, though it can progress to empyema.
  • Lung Abscess: A localized collection of pus within the lung parenchyma itself, not the pleural space. Imaging shows a cavitary lesion within the lung.
  • Malignant Pleural Effusion: Often an exudative effusion, but typically without acute signs of infection (fever, leukocytosis, purulent fluid are usually absent). Cytology of pleural fluid is diagnostic.
  • Tuberculous Pleurisy: Characterized by chronic effusion, often associated with weight loss and night sweats. Diagnosed by positive TB tests (AFB stain, culture, PCR) from pleural fluid or biopsy.
