Purpose: Detect small effusions, identify loculations, differentiate solid from fluid components, and guide thoracentesis or chest tube placement.
Typical Findings: Complex fluid collections with hyperechoic strands, septations, swirling debris; absence of lung sliding.
Clinical Importance: Bedside, real-time guidance, no radiation exposure, excellent for defining fluid characteristics and safe access.
A. Lifestyle Modifications: Not directly therapeutic for acute empyema, but good nutrition, hydration, and avoidance of smoking are essential for recovery and overall lung health.
B. Preventive Measures: Pneumococcal and influenza vaccinations, prompt and adequate antibiotic treatment of pneumonia.
C. Medical Treatment
With early diagnosis and aggressive management (antibiotics and drainage), the prognosis for empyema is generally good, with most patients achieving complete recovery. However, mortality rates can be significant (up to 20%) in elderly, immunocompromised, or critically ill patients, or those with delayed diagnosis and treatment. Long-term lung function can be compromised in cases requiring decortication.
