Giant Cell Arteritis (GCA) is a chronic, granulomatous, systemic vasculitis primarily affecting medium-to-large sized arteries. It predominantly targets the branches of the external carotid artery, though it can involve the aorta and its major branches. Left untreated, it poses a severe risk of permanent visual impairment due to ischemic optic neuropathy.
The exact cause is idiopathic, but it is considered an autoimmune condition. Genetic predisposition, specifically human leukocyte antigen (HLA) class II alleles (DRB1*04), plays a role. Environmental triggers, such as infections, may initiate an immune response in genetically susceptible individuals.
GCA involves T-cell-mediated immune responses. Dendritic cells in the adventitia of arterial walls trigger T-cell activation, releasing interferon-gamma and cytokines. This leads to the formation of multinucleated giant cells, intimal hyperplasia, and vessel wall thickening, causing luminal stenosis and tissue ischemia.
GCA exclusively affects adults over age
Age >50, Caucasian ethnicity, genetic susceptibility, family history, and concomitant Polymyalgia Rheumatica (PMR).
A. Early Symptoms
Headache (often localized to the temple), malaise, fatigue, low-grade fever. B. Common Symptoms
Scalp tenderness, jaw claudication (pain while chewing), weight loss, neck pain. C. Advanced Symptoms
Diplopia (double vision), blurred vision, systemic inflammatory signs. D. Emergency Symptoms

Physical findings include palpable, tender, or nodular temporal arteries, reduced or absent temporal pulses, and bruits over the carotid or subclavian arteries.

C. Medical Treatment: Glucocorticoids (Prednisone) are the mainstay. Tocilizumab (IL-6 inhibitor) is used as a corticosteroid-sparing agent.
D. Surgical Treatment: Rare, for aortic aneurysm repair.
E. Interventional Procedures: Temporal artery biopsy.
F. Rehabilitation: Physical therapy for corticosteroid-induced myopathy.
G. Emergency Management: High-dose intravenous methylprednisolone pulse therapy.
Good if diagnosed early; however, recurrence is common, requiring long-term monitoring.
