Granulomatosis with Polyangiitis (GPA) is a systemic, necrotizing granulomatous vasculitis primarily affecting small-to-medium-sized blood vessels. It is characterized by the triad of upper and lower respiratory tract involvement and glomerulonephritis.
The exact cause remains idiopathic. It is considered an autoimmune process triggered by an interplay between genetic predisposition (e.g., HLA-DPB10401) and environmental factors such as silica exposure, infectious agents, and chronic nasal carriage of Staphylococcus aureus*.
GPA is mediated by Anti-Neutrophil Cytoplasmic Antibodies (ANCAs), specifically anti-PR3 (Proteinase 3).
