Hemorrhoids are normal vascular cushions located within the anal canal, composed of arteriovenous anastomoses, smooth muscle, and connective tissue. When these cushions become abnormally dilated, inflamed, or displaced, they present as a pathological disease state. They are broadly classified as internal (originating above the dentate line, covered by columnar epithelium) or external (originating below the dentate line, covered by squamous epithelium).
The primary cause is increased intra-abdominal pressure and venous congestion. Contributing factors include chronic constipation or diarrhea, prolonged straining during defecation, a low-fiber diet, pregnancy (due to hormonal changes and pelvic pressure), obesity, prolonged sitting, and the age-related degeneration of supportive connective tissues (Treitz's muscle).
Pathological hemorrhoids develop when the supportive connective tissue framework anchoring the vascular cushions to the underlying internal anal sphincter disintegrates. This leads to abnormal downward displacement, venous engorgement, and mucosal prolapse. Straining causes venodilatation and mucosal microtrauma, manifesting as painless, bright red bleeding from the presinusoidal arterioles. External hemorrhoidal veins can thrombose, causing acute, severe pain due to stretching of the highly innervated anoderm.
Hemorrhoids are highly prevalent globally, affecting approximately 4.4% to 5% of the general population. Peak incidence occurs between 45 and 65 years of age. There is no significant gender predilection, though pregnancy significantly increases transient rates in women.

Colonoscopy Purpose: To evaluate the proximal colon. Typical Findings: Normal colonic mucosa, ruling out proximal pathology. Clinical Importance: Indicated in patients $\ge$ 45 years or those with red-flag symptoms (weight loss, iron deficiency anemia) to exclude malignancy.
The prognosis is excellent. Most early-stage hemorrhoids respond well to conservative therapies. Minimal intervention procedures have low recurrence rates (10–15%), while surgical excision provides a definitive cure in over 95% of cases, albeit with higher post-operative pain.
