Hypercalcemia is a metabolic disorder characterized by serum calcium concentrations above the upper limit of normal (>10.5 mg/dL or >2.62 mmol/L; ionized calcium >1.35 mmol/L). It disrupts cellular function across multiple organ systems, most notably the cardiac, renal, gastrointestinal, and central nervous systems.
Hypercalcemia occurs when calcium influx into the extracellular fluid (via osteoclastic bone resorption, intestinal absorption, or renal reabsorption) outpaces renal clearance. Normally, parathyroid hormone (PTH) and calcitriol maintain tight calcium homeostasis. High extracellular calcium activates calcium-sensing receptors (CaSR) in the kidney, which impairs renal concentrating mechanisms (causing nephrogenic diabetes insipidus) and triggers afferent arteriolar vasoconstriction, lowering the glomerular filtration rate (GFR).
Serum Total Calcium
Type: Blood Test
Purpose: Screen for and confirm elevated systemic calcium levels.
Expected Findings: >10.5 mg/dL (>2.62 mmol/L).
Interpretation: Elevated; must adjust for low albumin levels using the formula: Corrected Calcium = Total Calcium + [0.8 × (4.0 - Albumin)]. Intact PTH (iPTH)
Type: Blood Test
Purpose: Differentiate PTH-mediated causes from non-PTH-mediated causes.
Expected Findings: Elevated or inappropriately normal in PHPT; suppressed (<20 pg/mL) in malignancy and other causes.
Interpretation: High/normal points to PHPT or FHH; low suggests malignancy, vitamin D toxicity, or granulomatous disease. 24-Hour Urine Calcium
Type: Urine Test
Purpose: Differentiate PHPT from FHH.
Expected Findings: >200 mg/day in PHPT; low (<100 mg/day) in FHH.
Interpretation: Calcium-to-creatinine clearance ratio <0.01 indicates FHH; >0.02 indicates PHPT.
