Disease Overview

Intussusception is a serious gastrointestinal condition characterized by the telescoping of one segment of the intestine (the intussusceptum) into an adjacent, more distal segment (the intussuscipiens). This causes mechanical bowel obstruction, venous and lymphatic congestion, and, if untreated, arterial compromise leading to intestinal ischemia, necrosis, and perforation.

Etiology & Causes

  • Pediatric (Idiopathic): Up to 90% of pediatric cases have no identifiable lead point. It is often linked to viral infections (e.g., adenovirus) causing hypertrophy of Peyer's patches in the terminal ileum.
  • Pathological Lead Points: Meckel's diverticulum, intestinal polyps, duplication cysts, Henoch-Schönlein purpura hematomas, or lymphoma.
  • Adult Etiology: Rare (~5% of cases). Almost always secondary to a pathological lead point, primarily benign or malignant neoplasms (e.g., adenocarcinoma, lipoma).

Pathophysiology

The telescoping process begins when a segment of the bowel is swept forward by normal peristaltic activity. As the intussusceptum invaginates into the intussuscipiens, it pulls its mesentery along. This constricts mesenteric blood vessels, causing venous obstruction, edema, mucosal ischemia, and hemorrhage. The resulting mucosal sloughing, mixed with mucus, creates the classic "currant jelly" stool. Unrelieved pressure leads to arterial insufficiency, tissue necrosis, peritonitis, and bowel perforation.

  • Abdominal Plain Radiograph: Shows signs of bowel obstruction (dilated bowel loops, air-fluid levels) or free air under the diaphragm in cases of perforation.
  • Contrast or Air Enema: Both diagnostic and therapeutic. Shows a classic "claw sign" or "filling defect."

  • Volvulus: Rapid progression of bilious vomiting, identified by a "whirlpool sign" on ultrasound or malrotation on upper GI contrast series.
