A liver abscess is a localized, pus-filled cavity within the hepatic parenchyma. It is primarily caused by bacterial (pyogenic), parasitic (amebic), or rarely fungal invasions. If left untreated, the condition is uniformly fatal; however, modern diagnostic imaging, percutaneous drainage, and targeted antimicrobial therapies have significantly reduced mortality rates.
Pyogenic Abscess (80% of cases): Caused by bacterial pathogens. In Western countries, Escherichia coli and Bacteroides species are common, while Klebsiella pneumoniae* is highly prevalent in Asian populations.
Amebic Abscess (10% of cases): Caused by the protozoan Entamoeba histolytica*, acquired via the fecal-oral route.
Fungal Abscess (<10% of cases): Usually Candida* species, occurring in severely immunocompromised hosts.
Microorganisms reach the liver and overcome hepatic kupffer cells, leading to localized inflammatory cell infiltration. The release of bacterial toxins and neutrophil lysosomal enzymes causes liquefactive necrosis of hepatocytes. This necrotic tissue is walled off by a fibrous capsule, forming an abscess cavity. In amebic infections, the trophozoites cause enzymatic cytolysis, producing a classic "anchovy paste-like" fluid lacking neutrophils unless secondary bacterial infection occurs.

Type: Blood Test
Purpose: To detect antibodies against Entamoeba histolytica.
Expected Findings: Positive IgG antibodies.
Interpretation: High sensitivity for distinguishing amebic from pyogenic etiology in clinical contexts.
Abdominal Ultrasound: Purpose: Initial screening modality of choice. Typical Findings: Hypoechoic or mixed-echogenic round lesions with poorly defined borders and acoustic enhancement. Clinical Importance: Highly accessible, fast, and can guide immediate diagnostic aspiration.
Contrast-Enhanced CT (CECT) of the Abdomen: Purpose: Gold standard for definitive diagnosis, staging, and intervention planning. Typical Findings: Well-demarcated, low-attenuation (hypodense) lesion with a peripheral contrast-enhancing ring ("double-target sign"). Clinical Importance: Identifies multilocular abscesses, gas-fluid levels, and co-existing intra-abdominal pathology.
With early diagnosis, intravenous antibiotics, and percutaneous drainage, the survival rate is high, with mortality falling below 5–10%. Untreated pyogenic liver abscesses carry a mortality rate approaching 100%. Amebic abscesses have a highly favorable prognosis (<1% mortality) with metronidazole therapy alone.
