Microscopic Polyangiitis (MPA) is a systemic, necrotizing small-vessel vasculitis characterized by a lack of significant immune complex deposition (pauci-immune). It primarily affects the kidneys and lungs, frequently leading to rapidly progressive glomerulonephritis and pulmonary capillaritis.
The exact cause remains idiopathic. It is classified as an autoimmune condition where the immune system attacks small blood vessels. Environmental triggers, such as silica dust or specific drugs (e.g., hydralazine, propylthiouracil), have been associated with its onset. Genetic predisposition involves HLA-DR subtypes, though no singular causative gene exists.
MPA is driven by antineutrophil cytoplasmic antibodies (ANCA), primarily directed against myeloperoxidase (MPO). These antibodies activate neutrophils, causing them to adhere to the vascular endothelium, degranulate, and release reactive oxygen species and proteolytic enzymes. This leads to vessel wall necrosis, fibrinoid deposits, and end-organ ischemia, notably in the glomeruli and pulmonary microvasculature.
MPA is rare, with an annual incidence of 2–10 per million. It typically presents in the 5th to 6th decade of life. There is a slight male predominance, and it is more common in Caucasians than Asian populations.
Age (older than 50), prior exposure to silica, chronic infections, and specific medications (hydralazine, levamisole-tainted cocaine).

B. Preventive Measures: Vaccination against influenza/pneumonia, bone protection (bisphosphonates).
C. Medical Treatment:
Historically fatal; with current immunosuppressive therapy, the 5-year survival rate is 70–80%. Prognosis depends heavily on the degree of renal impairment at diagnosis.
No primary prevention. Secondary prevention involves close monitoring for relapse via serial ANCA titers and urine protein levels.
