Optic neuritis (ON) is an inflammatory, demyelinating disorder of the optic nerve. It is characterized by acute, usually unilateral, painful vision loss. It is strongly associated with demyelinating diseases, particularly Multiple Sclerosis (MS), often serving as the initial clinical manifestation.
Etiology involves immune-mediated inflammatory processes. Major causes include MS (via T-cell mediated demyelination), neuromyelitis optica spectrum disorder (NMOSD), myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), infections (viral, syphilis, Lyme disease), and autoimmune vasculitis.
The process involves inflammatory infiltration of the optic nerve, leading to myelin sheath destruction (demyelination) or axonal loss. This disrupts saltatory conduction, causing delayed or blocked nerve signal transmission. In MS-related ON, focal areas of demyelination (plaques) occur, while in MOGAD/NMOSD, the process is often more severe and necrotizing.
Most common in women aged 20–4
Female sex, Caucasian ethnicity, genetic predisposition (HLA-DRB1*15:01 allele), Epstein-Barr virus exposure, and vitamin D deficiency.
A. Early Symptoms
Inspection reveals an afferent pupillary defect (Marcus Gunn pupil). Ophthalmoscopy may show optic disc edema (papillitis) or appear normal (retrobulbar neuritis).
A. Clinical Assessment: History of pain on eye movement; swinging flashlight test.
B. Laboratory Testing: ESR, CRP, ANA, anti-AQP4, anti-MOG, syphilis serology.
C. Imaging Studies: Brain and orbit MRI with gadolinium.
D. Functional Tests: Visual Evoked Potentials (VEP).
E. Biopsy Findings: Generally not performed.
F. Genetic Testing: Not standard.
G. Differential Diagnosis: Ischemic optic neuropathy, toxic optic neuropathy.

Purpose: Identify NMOSD
Expected Findings: Positive
Interpretation: Highly specific for neuromyelitis optica.
MRI Brain/Orbits: Contrast-enhanced MRI typically shows hyperintense optic nerve signal on T2/STIR sequences and enhancement post-gadolinium. Used to assess MS risk via white matter lesions.
Optic atrophy, permanent visual field defects, conversion to clinically definite MS.
A. Lifestyle Modifications: Vitamin D supplementation.
B. Preventive Measures: Early initiation of Disease Modifying Therapies (DMTs) if MRI indicates high MS risk.
C. Medical Treatment
Most patients experience significant visual recovery within 2–6 months. Residual contrast sensitivity or color vision deficits are common.
Secondary prevention focuses on DMTs for underlying MS.
