Polyarteritis Nodosa (PAN) is a rare, systemic necrotizing vasculitis that primarily affects medium-sized muscular arteries. It leads to inflammation, tissue necrosis, and organ damage due to ischemia. Unlike ANCA-associated vasculitis, PAN does not involve capillaries, venules, or arterioles, sparing the lungs.
The exact cause remains idiopathic in most cases. Approximately 10–30% of cases are associated with chronic Hepatitis B virus (HBV) infection, triggering an immune-complex mediated response. Genetic factors are less defined compared to other vasculitides.
PAN involves segmental inflammation of medium-sized arteries. Immune complex deposition in vessel walls triggers complement activation, neutrophil infiltration, and fibrinoid necrosis. This results in luminal narrowing, thrombosis, and the formation of characteristic microaneurysms, leading to distal ischemia and end-organ infarction.

Chronic Hepatitis B infection, male gender, advanced age, and exposure to certain drugs (e.g., sulfonamides) are identified triggers.
A. Early Symptoms: Unexplained weight loss, malaise, low-grade fever, myalgia, arthralgia.
B. Common Symptoms: Abdominal pain (post-prandial), skin nodules, livedo reticularis, hypertension.
C. Advanced Symptoms: Peripheral neuropathy (mononeuritis multiplex), digital gangrene, renal insufficiency.
D. Emergency Symptoms: Bowel perforation, myocardial infarction, intracranial hemorrhage, acute renal failure.
Hypertension is common due to renal artery involvement. Palpation may reveal tender subcutaneous nodules or ulcers. Neurological exams often show asymmetrical distal weakness (foot drop or wrist drop).

Without treatment, the 5-year survival rate is <15%; with treatment (steroids/immunosuppressants), survival reaches 80-90%.
