Polycythemia Vera (PV) is a rare, chronic myeloproliferative neoplasm characterized by the overproduction of red blood cells, and often white blood cells and platelets, by the bone marrow. This leads to an increased blood volume and viscosity, increasing the risk of thrombotic events and bleeding.
The exact cause of PV is largely unknown. The majority of cases (over 95%) are acquired and associated with a somatic mutation in the Janus kinase 2 (JAK2) gene, specifically the JAK2 V617F mutation. Rare cases may be due to other JAK2 mutations or mutations in other genes like CALR or MPL. It is not considered a hereditary disease.

Polycythemia Vera is distinguished from secondary causes of erythrocytosis by the presence of JAK2 mutation, leukocytosis, thrombocytosis, and often splenomegaly. Secondary erythrocytosis can be caused by hypoxia (lung disease, high altitude, sleep apnea), kidney tumors, or the use of erythropoietin-stimulating agents. Other myeloproliferative neoplasms can have overlapping features.

Primary prevention is not possible as PV is caused by an acquired genetic mutation. Secondary prevention focuses on preventing thrombotic and hemorrhagic complications through regular monitoring and appropriate medical treatment (e.g., phlebotomy, low-dose aspirin, cytoreductive therapy).
