Etiology & Causes

The exact cause of PV is largely unknown. The majority of cases (over 95%) are acquired and associated with a somatic mutation in the Janus kinase 2 (JAK2) gene, specifically the JAK2 V617F mutation. Rare cases may be due to other JAK2 mutations or mutations in other genes like CALR or MPL. It is not considered a hereditary disease.

In PV, hematopoietic stem cells acquire a mutation (most commonly JAK2 V617F) that leads to their uncontrolled proliferation. This results in an increased production of erythrocytes, granulocytes, and platelets.
