Polymyalgia rheumatica (PMR) is an inflammatory rheumatic condition characterized by profound pain and stiffness in the shoulders, hip girdles, and neck. It predominantly affects individuals over the age of 50 and is strongly associated with giant cell arteritis (GCA).
The exact cause is unknown, but it is believed to result from an interaction between genetic predisposition (specifically HLA-DRB1 alleles) and environmental triggers, such as viral infections, which initiate an autoimmune response in the synovial tissues and bursae.
PMR involves an inflammatory process in the extra-articular structures. Histopathology reveals synovitis, tenosynovitis, and bursitis, particularly in the subacromial and subdeltoid bursa. Cytokine dysregulation, primarily involving Interleukin-6 (IL-6), plays a central role in systemic inflammation.

Inspection may show limited range of motion (ROM) in shoulders/hips. Palpation reveals tenderness over the bursae. Neurological examination is typically normal (no primary muscle weakness, only pain-limited effort).
