Prader-Willi Syndrome (PWS) is a complex, multisystem genetic disorder characterized by neonatal hypotonia, hyperphagia leading to obesity, cognitive impairment, and distinct behavioral challenges. It results from the loss of function of specific genes on the paternal chromosome 15q11.2-q13 region.
PWS is caused by the absence of paternal expression of genes in the 15q11.2-q13 region. This occurs through three primary mechanisms: paternal chromosomal deletion (65–75%), maternal uniparental disomy (20–30%), or imprinting defects (1–3%).
The defect involves genes involved in hypothalamic development and function. Dysregulation of the hypothalamic-pituitary axis leads to growth hormone deficiency, hypogonadism, and impaired satiety signaling (ghrelin dysregulation), culminating in the hallmark insatiable hunger.
PWS affects approximately 1 in 15,000 to 30,000 live births. There is no significant predilection based on gender or ethnicity.
