Preeclampsia is a pregnancy-specific multisystem disorder characterized by new-onset hypertension and proteinuria or evidence of end-organ dysfunction after 20 weeks of gestation. It is a leading cause of maternal and neonatal morbidity worldwide.
The exact cause remains unknown but is rooted in abnormal placentation. It involves an imbalance between angiogenic and anti-angiogenic factors (e.g., sFlt-1 and PlGF) leading to systemic endothelial dysfunction. Genetic predisposition, maternal immunologic intolerance to fetal allografts, and systemic inflammation contribute to its development.
Incomplete remodeling of maternal spiral arteries leads to high-resistance, low-flow placental perfusion. This results in placental ischemia, triggering the release of inflammatory factors into maternal circulation, causing generalized endothelial cell activation, vasospasm, increased vascular permeability, and activation of the coagulation cascade.
Preeclampsia affects 3–5% of all pregnancies. It is more common in nulliparous women and those with a personal or family history. Prevalence is higher in regions with limited access to prenatal care and among populations with higher rates of chronic metabolic diseases.
Nulliparity, advanced maternal age (>35), obesity, chronic hypertension, pre-gestational diabetes, chronic kidney disease, antiphospholipid syndrome, and multiple gestations.
A. Early Symptoms: Mild headache, slight peripheral edema, steady weight gain.
B. Common Symptoms: Persistent headache, edema of the face and hands, nausea/vomiting.
C. Advanced Symptoms: Visual disturbances (blurred vision, scotomata), epigastric or right upper quadrant (RUQ) abdominal pain, dyspnea.
D. Emergency Symptoms: Seizures (eclampsia), loss of consciousness, severe intractable headache, oliguria/anuria.

Low-dose aspirin therapy for high-risk patients, calcium supplementation in deficient populations, and early initiation of prenatal care.
