A prolactinoma is a benign, prolactin-secreting epithelial tumor (adenoma) of the lactotroph cells in the anterior pituitary gland. It is the most common type of functioning pituitary hormone-producing tumor, leading to hyperprolactinemia. This excess prolactin disrupts the hypothalamic-pituitary-gonadal axis, presenting primarily as reproductive dysfunction, galactorrhea, and, in the case of larger tumors, compressive neurological mass effects.

Etiology & Causes

Prolactinomas arise from the monoclonal expansion of pituitary lactotrophs. The vast majority of cases are sporadic, with no identifiable environmental or lifestyle causes. A small percentage (approximately 5%) are associated with hereditary syndromes, primarily Multiple Endocrine Neoplasia Type 1 (MEN1) due to mutations in the MEN1 gene, or familial isolated pituitary adenomas (FIPA) associated with AIP gene mutations.

Pathophysiology

  • Hormonal Hypersecretion: Lactotroph adenomas autonomously secrete prolactin. Elevated serum prolactin levels exert negative feedback on the hypothalamus, stimulating dopamine release and suppressing Gonadotropin-Releasing Hormone (GnRH) pulsatility.
  • Gonadal Axis Suppression: Decreased GnRH secretion leads to reduced Luteinizing Hormone (LH) and Follicle-Stimulating Hormone (FSH) secretion from the gonadotrophs, causing hypogonadotrophic hypogonadism (manifesting as oligomenorrhea/amenorrhea in women and low testosterone/erectile dysfunction in men).
  • Mass Effect: Tumors are classified as microadenomas (<10 mm) or macroadenomas (≥10 mm). Macroadenomas can expand superiorly to compress the optic chiasm, leading to visual field deficits, or laterally into the cavernous sinuses, affecting cranial nerves III, IV, and VI.

Serum Prolactin Type: Blood Test


  • Purpose: Direct measurement of prolactin secretion.

  • Interpretation: High levels confirm hyperprolactinemia. Note: "Hook effect" can falsely lower values in giant macroadenomas; serial dilution is required.


Beta-hCG Type: Blood Test

  • Purpose: Rule out pregnancy as a physiological cause of hyperprolactinemia.

  • Expected Findings: Negative.

  • Interpretation: Excludes pregnancy.


TSH and Free T4 Type: Blood Test

  • Purpose: Rule out primary hypothyroidism (where TRH elevations stimulate lactotrophs).

  • Expected Findings: Normal thyroid panel.

  • Interpretation: Rules out secondary cause of prolactin elevation.

  • Clinical Importance: Distinguishes between microadenoma (<10 mm) and macroadenoma (≥10 mm), and assesses optic chiasm or cavernous sinus involvement.

  • Non-Functioning Pituitary Adenoma (Stalk Effect): Mass compresses pituitary stalk, blocking dopamine transit. Prolactin is elevated but usually <100–150 ng/mL.
  • Drug-Induced Hyperprolactinemia: Antipsychotics (e.g., risperidone), metoclopramide, and verapamil block dopamine. Prolactin levels rarely exceed 200 ng/mL.
  • Primary Hypothyroidism: Elevated TRH stimulates prolactin release. Resolved with levothyroxine.
