Scleritis is a severe, chronic, and potentially vision-threatening inflammatory disease involving the sclera (the white, fibrous outer coat of the eye). Unlike episcleritis, scleritis is characterized by deep-seated pain and significant inflammation that often heralds underlying systemic autoimmune disease.
Scleritis is largely considered an immune-mediated condition. Etiologies include systemic autoimmune diseases (rheumatoid arthritis, granulomatosis with polyangiitis, relapsing polychondritis), infectious causes (herpes zoster, Pseudomonas, tuberculosis, syphilis), and localized trauma or post-surgical complications.
The pathophysiology involves immune complex deposition or Type IV hypersensitivity reactions within the scleral collagen fibers. Inflammatory mediators (cytokines, prostaglandins) trigger localized vasodilation and edema. Chronic inflammation leads to the activation of fibroblasts, resulting in tissue thinning, necrosis, and scleral melting.
Scleritis is relatively rare, with an incidence of approximately 4.7 per 100,000 person-years. It typically affects adults aged 40–60 years and demonstrates a predilection for women, particularly in the context of associated connective tissue diseases.
A. Early Symptoms
Inspection reveals deep, violaceous redness. Palpation of the globe elicits extreme tenderness. Slit-lamp examination demonstrates scleral edema and vascular congestion.
