Strabismus is a clinical condition characterized by the ocular misalignment of the visual axes, where the eyes do not point at the same object simultaneously. This misalignment disrupts binocular vision, potentially leading to amblyopia (lazy eye) and impaired depth perception.
Etiology involves disruption of the binocular coordination centers in the brain or mechanical dysfunction of extraocular muscles. Causes include neurological disorders (cerebral palsy, hydrocephalus), trauma, uncorrected refractive errors (hyperopia), and genetic predispositions involving familial patterns of ocular muscle control.
Pathophysiology involves a failure of the cortical mechanisms (vergence) to maintain ocular alignment. This may arise from primary muscle weakness or, more commonly, dysfunction in the cranial nerves (III, IV, VI) or the supranuclear pathways. If onset occurs during the critical period of visual development, the brain suppresses input from the misaligned eye, leading to irreversible amblyopia.
Strabismus affects approximately 2% to 4% of the global population. Onset is most common in early childhood (infantile or accommodative esotropia), though secondary strabismus can develop at any age due to neurological injury or orbital disease.

Inspection reveals ocular deviation. The Hirschberg test (corneal light reflex) shows asymmetric light reflection. The cover-uncover test demonstrates the shift of the fixating eye when the deviating eye is covered.
