Thrombotic Thrombocytopenic Purpura (TTP) is a rare, life-threatening hematological emergency characterized by the formation of extensive microthrombi in small blood vessels. This process leads to profound thrombocytopenia (low platelet count), microangiopathic hemolytic anemia (MAHA), and varying degrees of organ ischemia, most commonly affecting the brain, heart, and kidneys. The condition is primarily caused by a deficiency of the enzyme ADAMTS13, which is responsible for cleaving large von Willebrand factor (VWF) multimers. Without this cleavage, VWF-rich clots spontaneously form. TTP requires immediate intervention, as it has a mortality rate exceeding 90% if left untreated, which can be reduced to approximately 10-20% with modern therapy.
TTP is classified into two primary forms based on the cause of ADAMTS13 deficiency:
The pathophysiology centers on the "ultralarge" von Willebrand factor (ULVWF) multimers. Normally, the metalloprotease ADAMTS13 cleaves these large multimers into smaller, less prothrombotic pieces. When ADAMTS13 activity is absent (<10% of normal), these ULVWF multimers persist and bind excessively to platelets, especially under high shear stress in the microcirculation. This results in:

Expected Findings: Presence of schistocytes (fragmented RBCs).

Expected Findings: Activity level <10%.
Interpretation: Confirms the diagnosis of TTP. Lactate Dehydrogenase (LDH)
Type: Blood Test
Purpose: To assess for hemolysis and tissue ischemia.
Expected Findings: Significantly elevated.
Interpretation: Indicates high cell turnover and hemolysis severity. Indirect Bilirubin & Haptoglobin
Type: Blood Test
Purpose: To confirm hemolytic anemia.
Expected Findings: Elevated indirect bilirubin; undetectable haptoglobin.
Interpretation: Confirms intravascular hemolysis.
