Uterine fibroids are benign, monoclonal tumors arising from the smooth muscle cells of the myometrium. They are the most common pelvic tumors in reproductive-aged women, frequently causing abnormal uterine bleeding, pelvic pain, and infertility, though many remain asymptomatic.
Etiology involves a combination of genetic predispositions and hormonal factors. Fibroids are estrogen and progesterone-dependent, as evidenced by their growth during reproductive years and regression after menopause. Genetic mutations, notably in the MED12 gene, are frequently identified.
Pathophysiology involves dysregulated cell proliferation and excessive extracellular matrix (ECM) deposition. Increased concentrations of growth factors (TGF-β, FGF) and estrogen receptors lead to an enlarged, distorted myometrial architecture.
