Von Willebrand disease (VWD) is the most common inherited bleeding disorder, characterized by a deficiency or dysfunction of von Willebrand factor (VWF). VWF is a crucial protein in blood clotting, facilitating platelet adhesion to injured blood vessel walls and acting as a carrier for Factor VIII. This deficiency leads to a tendency for prolonged bleeding after injury or surgery, and spontaneous bleeding episodes.

Test Name: Complete Blood Count (CBC)
Type: Blood Test
Purpose: To assess for anemia due to chronic blood loss and to evaluate platelet count.

Interpretation: Anemia is a common consequence of VWD. Test Name: Prothrombin Time (PT)
Type: Blood Test
Purpose: To assess the extrinsic and common pathways of coagulation.
Expected Findings: Typically normal in VWD.
Interpretation: Normal PT helps differentiate VWD from factor deficiencies in the extrinsic pathway (e.g., Factor VII deficiency). Test Name: Activated Partial Thromboplastin Time (aPTT)

Purpose: Measures the amount of VWF protein in the blood.
Expected Findings: Reduced in Type 1 and Type 3 VWD, normal or elevated in Type 2 VWD.
Interpretation: Low VWF:Ag is indicative of a quantitative defect. Test Name: Von Willebrand Factor Activity (VWF:RCo or VWF:CB)
Type: Blood Test
Purpose: Measures the functional capacity of VWF to bind to platelets (ristocetin cofactor activity) or collagen.
Expected Findings: Reduced in all types of VWD. The reduction is often more pronounced for VWF:RCo than VWF:Ag in Type 2 VWD.
Interpretation: Reduced VWF activity confirms a functional defect, even if the VWF antigen level is normal or high. Test Name: Factor VIII Activity (FVIII:C)
Type: Blood Test
Purpose: Measures the activity of Factor VIII.
Expected Findings: Can be reduced in VWD, particularly in Type 1 and Type 3, due to impaired VWF protection.
Interpretation: Low FVIII:C can contribute to the bleeding phenotype and may require treatment with Factor VIII concentrate.
