Whipple's disease is a rare, systemic bacterial infection caused by Tropheryma whipplei. It primarily targets the gastrointestinal tract, causing severe malabsorption, but can affect any organ system, including the joints, central nervous system, and cardiovascular system.
The sole causative agent is Tropheryma whipplei, a Gram-positive, rod-shaped actinobacterium. Genetic predisposition (specifically HLA-B27 association) and subtle immunologic defects in macrophage function allow the bacterium to replicate intracellularly. Environmental exposure, particularly to soil and wastewater, is common.
T. whipplei invades the small intestinal mucosa, where it is phagocytosed by macrophages. These macrophages fail to degrade the bacteria, leading to massive accumulation in the lamina propria. This causes lymphatic obstruction, blunting of intestinal villi, and impaired lipid transport, leading to severe malabsorption and systemic dissemination via lymphatics and blood.

A. Clinical Assessment: History of migratory joint pain followed by chronic diarrhea and weight loss.

No primary prevention exists. Secondary prevention relies on completing the full 12-month course of antibiotics and periodic clinical follow-up.
