The sole causative agent is Tropheryma whipplei, a Gram-positive, rod-shaped actinobacterium. T. whipplei invades the small intestinal mucosa, where it is phagocytosed by macrophages. These macrophages fail to degrade the bacteria, leading to massive accumulation in the lamina propria.
