Psoriatic arthritis (PsA) is a chronic, progressive inflammatory skeletal disorder that occurs in approximately 30% of patients diagnosed with psoriasis. It is classified as a seronegative spondyloarthropathy, characterized by joint inflammation, enthesitis (inflammation where tendons/ligaments attach to bone), dactylitis ("sausage digits"), and potential axial involvement. While skin symptoms typically precede joint symptoms by several years, in about 15% of cases, arthritis appears first or concurrently with skin lesions.
The exact cause of PsA is unknown, but it results from a complex interplay between genetic predisposition, environmental triggers, and immune system dysfunction.
PsA is characterized by synovitis, enthesitis, and abnormal bone remodeling. Unlike rheumatoid arthritis, which primarily affects the synovium, PsA often begins at the enthesis.

Ultrasound (Musculoskeletal): Purpose: Visualize soft tissue inflammation. Findings:* Power Doppler signal showing increased blood flow at the enthesis.
The prognosis for PsA is variable. While some have a mild, remitting course, up to 20% develop a rapidly progressive, destructive form of arthritis. Early diagnosis and "treat-to-target" strategies using modern biologics have significantly improved outcomes, allowing many patients to achieve clinical remission and prevent permanent disability.
There is no known way to prevent the onset of PsA in susceptible individuals. However, secondary prevention involves early detection and aggressive treatment to prevent structural joint damage and functional loss.
