Comprehensive Diagnostic & Therapeutic Reference Profile
Also known as: HRS, Hepatorenal Kidney Injury, HRS-AKI, HRS-NAKI
Hepatorenal Syndrome (HRS) is a life-threatening, reversible form of functional renal failure occurring in patients with advanced chronic liver disease, liver failure, or portal hypertension. It is characterized by severe renal vasoconstriction leading to a drastic reduction in glomerular filtration rate (GFR) in the absence of primary histopathological kidney disease.
HRS is caused by severe portal hypertension resulting from liver cirrhosis, alcoholic hepatitis, or acute liver failure. Precipitating factors include:
The primary mechanism involves portal hypertension, which triggers arterial vasodilation in the splanchnic circulation, mediated primarily by nitric oxide. This vasodilation leads to a pooling of blood in the splanchnic bed and a decrease in effective arterial blood volume. In response, the body activates compensatory systems: the renin-angiotensin-aldosterone system (RAAS), the sympathetic nervous system (SNS), and antidiuretic hormone (vasopressin). While these systems maintain systemic blood pressure, they trigger profound, compensatory renal vasoconstriction. Over time, renal perfusion falls, lowering GFR and leading to functional renal failure.
The following homeopathic remedies have been historically indicated for symptoms associated with Hepatorenal Syndrome. Selection should be based on individualized symptom totality and constitutional assessment.
