Nephrotic syndrome is a clinical state characterized by a triad of massive proteinuria (>3.5 g/24 hours), hypoalbuminemia (<3.0 g/dL), and generalized edema. It occurs due to glomerular filtration barrier dysfunction, which leads to excessive urinary protein loss, hyperlipidemia, and lipiduria.
The primary defect is increased permeability of the glomerular filtration barrier, specifically damage to podocytes (slit diaphragms) and the glomerular basement membrane (GBM).
