Polycystic Kidney Disease (PKD) is a progressive genetic disorder characterized by the development and growth of numerous fluid-filled cysts within the renal parenchyma. These cysts cause massive bilateral kidney enlargement, parenchymal destruction, and eventual end-stage renal disease (ESRD).
PKD is primarily hereditary, caused by genetic mutations:
ADPKD (90% of cases): Autosomal dominant inheritance of mutations in PKD1 (chromosome 16, ~85% of cases, more severe) or PKD2* (chromosome 4, ~15% of cases).
ARPKD (rare): Autosomal recessive inheritance of mutations in the PKHD1* gene on chromosome

Mutations in the polycystin-1 (PKD1) or polycystin-2 (PKD2) proteins disrupt primary cilia function in renal tubular epithelial cells. This disruption alters intracellular calcium signaling, triggering unregulated cellular proliferation, fluid secretion, and extracellular matrix remodeling. Over time, expanding cysts compress surrounding nephrons and vasculature, causing local ischemia, interstitial fibrosis, and loss of functional nephrons.
Renal Ultrasound: Purpose: Primary diagnostic and screening tool. Typical Findings: Multiple fluid-filled, bilateral renal cysts. Diagnosis is confirmed based on age-dependent cyst count criteria. Clinical Importance: Highly sensitive, cost-effective, and avoids radiation.
Magnetic Resonance Imaging (MRI): Purpose: Disease monitoring and staging. Typical Findings: High-resolution visualization of renal parenchyma. Clinical Importance: Measures Total Kidney Volume (TKV) to predict the risk of progression to ESRD (Mayo Classification).
Variable. Approximately 50% of patients with ADPKD progress to ESRD by age
No primary prevention exists. Secondary prevention relies on rigorous blood pressure control, high hydration, use of Tolvaptan in eligible high-risk patients, and genetic counseling for affected families.
