Syringomyelia is a chronic, progressive neurological disorder characterized by the development of a fluid-filled cavity (syrinx) within the parenchyma of the spinal cord. Over time, this cavity expands, compressing and damaging adjacent spinal cord tracts. This damage leads to a classic clinical presentation of dissociated sensory loss, muscle weakness, and autonomic dysfunction.
Disruption of physiological cerebrospinal fluid (CSF) flow dynamics at the craniocervical junction or spinal canal redirects CSF under pressure into the spinal cord parenchyma. Fluid accumulates in the extracellular space or central canal, creating a syrinx. Longitudinal expansion and progressive dilation of the syrinx cause mechanical compression, ischemia, and subsequent gliosis of the decussating spinothalamic fibers in the anterior white commissure, followed by anterior horn cells and corticospinal tracts.
MRI of the Spine (Cervical/Thoracic): Purpose: Primary diagnostic modality. Typical Findings: Well-demarcated, intramedullary cavity matching CSF signal intensity (hypointense on T1, hyperintense on T2). Clinical Importance: Defines syrinx dimensions, localization, and detects associated pathology (e.g., Chiari malformation, spinal tumors).
Cine-MRI (Phase-Contrast): Purpose: Evaluate CSF flow dynamics. Typical Findings: Abnormal CSF flow patterns at the craniocervical junction. Clinical Importance: Aids in surgical planning and assessing postoperative decompression success.
