Etiology & Causes

Primary VUR is most often due to an anatomical abnormality where the ureterovesical junction (UVJ), the point where the ureter enters the bladder, is underdeveloped or malformed. This can result in a shortened intravesical tunnel of the ureter, compromising its ability to close effectively during bladder contraction. Secondary VUR results from conditions that obstruct urine outflow from the bladder or impair its normal emptying, such as posterior urethral valves, bladder neck obstruction, or neurogenic bladder.

The normal physiological mechanism preventing VUR involves the length and obliquity of the intravesical ureteral tunnel. As the bladder fills and contracts, increased intravesical pressure compresses this tunnel, effectively closing the ureteral orifice and preventing urine reflux. In primary VUR, a congenitally short or inadequately angled tunnel allows urine to flow back into the ureter when intravesical pressure exceeds the resistance at the UVJ. This stagnant urine in the ureters and renal pelvis creates a breeding ground for bacteria, leading to recurrent UTIs. Persistent or severe reflux can cause hydronephrosis (swelling of the kidney due to urine backup) and renal parenchymal scarring, which can impair kidney function and lead to hypertension and chronic kidney disease (CKD).
